Use this form to opt your student out of surveys, evaluations, questionnaires, curriculum, programs, celebrations, religious observances, transgender ideology, counseling, and medical treatments that do not align with your values or the United States Constitution. As a parent/guardian, you have the fundamental right to direct the education, mental health, and health care of your children.
